Medical Requirements - Please read before booking

Scuba Diving is physically demanding and can be dangerous if you suffer from certain medical conditions. It is very important that you, the diver, do not suffer from any contraindications to diving.

All divers must read and complete our medical declaration questionnaire & receive an accredited medical clearance from a Qualified Dive Physician before booking their Shark Dive - Download form here. 

(This is only required if you have ticked Yes to any of the conditions listed).

If you are booking a dive or purchasing a gift voucher for another person, please ensure the recipient is aware of the medical requirements. 

 

COVID-19

If you have suffered from COVID-19 in the past 3 months, please read and complete this Shark Dive Xtreme COVID-19 medical questionnaire before booking -  Download the COVID-19 form here.

 

IMPORTANT NOTICE - Conditions that may prevent you from diving

If you suffer from any of the below medical conditions, then you will be required to obtain a fully completed medical clearance from Qualified Dive Physician before booking your dive;

Please note: If you are pregnant (or planning to become pregnant), you will not be permitted to dive.

 

  • Issues with lungs, breathing, heart or blood (affecting physical or mental performance)
  • Struggle to perform moderate exercise (eg. walking 1.6 kilometres or swimming 200 metres without resting) OR have been unable to participate in normal physical activity due to fitness or health reasons within the past 12 months
  • Have issues with eyes, ears or nasal passages/sinuses
  • Have had surgery within the last 12 months, or have ongoing problems related to past surgery
  • Have lost consciousness, had migraine headaches, seizures, strokes, significant head injury, or suffer from persistent neurologic injury or disease
  • Currently undergoing treatment (or have required treatment in the last five years) for psychological problems, personality disorder, panic attacks, or an addiction to drugs and alcohol; or have been diagnosed with a learning or developmental disability
  • Had back problems, hernia, ulcers or diabetes
  • Have had stomach or intestine problems, including recent diarrhea
  • Taking prescription medications (with the exception of birth control or anti-malarial drugs other than mefloquine (Lariam)

Please do not book your dive before you have obtained medical clearance.

FAQs

This is an intimate experience with only 4 people per dive, so sessions often book out in advance. To avoid missing out, we recommend booking online.

We will endeavor to get back to all enquiries within 24 hours, Tuesday to Saturday.

You must be 14 years or older to participate in our Shark Dive Xtreme experience. A parent or guardian must accompany children under 18.

To book a date for your experience with a gift voucher, please pre-book your timeslot. 

Please note, only Shark Dive Xtreme vouchers purchased via SEA LIFE Melbourne may be redeemed, any vouchers purchased through a third-party site must be redeemed through their respective website.

Guests will need to bring the following for their dive:

  • Swimming costume/swimmers
  • Enclosed footwear
  • A towel
  • Toiletries (you are welcome to use our showers after your dive)

No, but friends and family can take photos through the viewing tunnel. You can also purchase underwater souvenir photo and video footage after your dive.

Friends and family can watch from the safety of the viewing tunnel and wave, blow kisses and take photographs. Friends and family get a 25% off admission (up to 4 people per diver) to SEA LIFE Melbourne to watch you dive. When you purchase your Shark Dive Xtreme ticket, you will have the opportunity to purchase the discounted spectator ticket before you confirm your purchase.
